In order for the Housing Authority to process your request for housing, the following verification must accompany your application:

• Verification of your income
• Verification of your assets
• Birth Certificates for all members in your household
• Social Security Cards for all members in your household
• Disability verification (if applicable) -- letter from physician stating the nature and duration of your disability
• Discharge papers for Veteran Status (if applicable)
• Authorization for Release of Information (click here to download and print)

IMPORTANT:   The Housing Authority does send out landlord references.  It is extremely important that you give five (5) years landlord references (#19 on the application).   Please include full addresses (street and number, city, state and zip code).   If you do not have five years landlord references, you must write a letter stating the reason.
